Update Your Physician Listing
Please complete the form below to let us know about changes that you would like to make to your physician listing on this website or to the Health Line referral service. Your requested updates will be received by the medical staff office for verification and then updated in both the Find a Doctor directory and the Health Line physician referral service.
Some common update requests include:
- Office or Practice Name
- Board Certifications
- Medical School, Residency or Internship
- Insurances Accepted
- Whether You Are (or Aren't) Accepting New Patients
- Listing Not Found or Displayed Incorrectly
Update Personal Contact or Office Contact Information with the Medical Staff Office
To update your personal contact information with the Medical Staff Office, please download the Medical Staff Contact Information Change Form.
Once you complete and sign the form, please fax it to the El Camino Health Medical Staff Services Department at 650-966-9263 or email it to firstname.lastname@example.org. A medical staff representative will contact you to verify the changes.
