We make investments to meet our community’s most pressing needs by working closely with our partners. Together, we aim to improve access to care and programs that support healthy Highlights living for people of all ages.

Improved Access to Primary, Specialty and Preventive Care

2,069 individuals established care with a primary care provider or specialist

1,865 individuals received follow-up care after health screenings

855 individuals reported improved oral health after service

608 individuals were successfully connected to clinical or community services through care navigation support

Schools, community clinics, and nonprofit organizations supported community members, including high-risk groups such as the unhoused, individuals with disabilities, and underserved populations, by helping them establish care, access follow-up services, improve oral health, and connect with essential programs.

12,855 hours of counseling sessions were provided to youth

9,338 hours of counseling sessions were provided to adults

4,191 hours of training provided to program participants

Schools and community programs delivered over 26,000 hours of mental health counseling and training, providing trauma-informed care, peer support, and specialized services for youth, adults, educators, and individuals facing housing instability, behavioral health challenges, or other barriers to care.

Promoted Healthy Lifestyle Habits

1,002 adults reported 150 minutes of physical activity per week

4,943 youth reported 150 minutes of physical activity per week

2,599 individuals improved one or more biometric health indicator

School and community-based programs promoted regular physical activity among youth and adults, helping thousands meet CDC guidelines and improve key health indicators to reduce the risk of diabetes and obesity.

Improved Chronic Disease Management and Prevention

377 individuals reported improved self-management of their chronic health condition

Programs supported patients and community members in managing their chronic health conditions, with many reporting improved self-management through targeted education and support services.

Removed Barriers to Basic Needs

4,047 individuals were connected to a sustainable source of healthy food (CalFresh/SNAP, food bank, etc.)

500 individuals improved their living conditions as a result of services provided

A dedicated and diverse group of local nonprofit organizations connected thousands of community members to healthy food and helped individuals improve their living conditions, reducing barriers to basic needs for those experiencing economic hardship.
