At your Lifestyle Medicine Consultation
You will meet with one of our certified lifestyle medicine specialists for a 1-hour telehealth discussion to:
- Review your cardiac and metabolic risk profile
- Assess your lifestyle habits and behaviors that may increase your risk
- Identify areas you are motivated/eager to address
- Evaluate the areas where you may make changes to improve health and well-being
- Create a personalized plan to reduce risk or help manage chronic conditions
- Set goals, provide education, and offer support to make sustainable changes
Scheduling Your Appointment
Your out-of-pocket cost of this consultation is $119 and is FSA/HSA reimbursable. Once you complete your payment, we will send you the link to schedule your consultation.
Preparing for Your Lifestyle Medicine Consultation
- Complete the online Health Risk Assessment and Nutrition Journal. We will email the link and personalized access code to you separately.
- Provide recent laboratory results including your Lipid Panel, HbA1c, and Fasting Blood Glucose (FBS) and we will send you an email with instructions on sharing the results securely.
