COVID-19 vaccinations are only available to those who meet State criteria. Currently, we can vaccinate:

  1. anyone who is 12 years of age or older and lives or works in Santa Clara County; or
  2. patients who receive their primary care through El Camino Health.

If you are unsure if you meet the above criteria, you can find more information online.

Walk-ins are now permitted at our Sunnyvale location. Appointments are still strongly encouraged to help ensure your vaccination on the date and time of your preference.

To schedule a vaccination appointment, please first fill out the form below to see if you are eligible.

Do you live or work in Santa Clara County or receive your primary care from El Camino Health?
Have you received another vaccine (e.g. influenza) in the last 14 days?
Have you received antibody therapy (monoclonal antibodies or convalescent serum) to treat COVID-19 in the last 90 days?
Have you ever had an allergic reaction to: polysorbate, or a prior dose of the COVID-19 vaccine, or a component of the COVID-19 vaccine, including polyethylene glycol (PEG), which is found in some medications, such as laxatives and preparations for colonoscopy procedures? This would include a severe allergic reaction (e.g., anaphylaxis) that required treatment with epinephrine or EpiPen® or that caused you to go to the hospital. It would also include an allergic reaction that occurred within 4 hours that caused hives, swelling, or respiratory distress, including wheezing.
