
Primary Care Advanced Practice Provider (APP) (San Jose)

ECHMN is seeking a compassionate and highly skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Primary Care team in an outpatient ambulatory care setting. The APP will assesses, diagnose, plan and implement the medical care of patients in collaboration with a physician. Functioning within the full scope of licensure; the APP will be accountable for the assessment, coordination delivery and evaluation of care, including direct patient care. Provides medical consultation including the performance of therapeutic procedures delegated by the supervising physician, assist with various procedures, provides patient education, treatment, and follow-up. All APP’s will follow ambulatory care and inpatient standardized procedures, protocols, policy and procedures regulated by the State of California Physician Assistant Practice Act.
Key Responsibilities
- Provide comprehensive primary care services, including health assessments, diagnosis, treatment, and management of acute and chronic conditions.
- Perform routine physical exams, order and interpret diagnostic tests, and prescribe appropriate medications and treatments.
- Develop individualized care plans and collaborate with patients on preventive health strategies.
- Coordinate care with other healthcare professionals, specialists, and support staff as needed.
- Document all patient interactions accurately and timely in the Electronic Medical Record (EMR) system.
- Participate in quality improvement initiatives, clinical meetings, and continuing education programs.
- Provide patient education and counseling on health maintenance and disease prevention.
- Maintain compliance with organizational policies, clinical protocols, and state/federal regulations.
Qualifications
- Education:
- Master’s or Doctorate degree from an accredited Nurse Practitioner or Physician Assistant program.
- Licensure/Certification:
- Current and unrestricted state licensure as a Nurse Practitioner or Physician Assistant.
- National board certification (AANP, ANCC, or NCCPA).
- DEA registration and prescriptive authority.
- BLS-Basic Life Support Healthcare Provider
- Other - certification based on department requirements
- Experience:
- 2 years of relevant experience in a primary care, family practice, or internal medicine setting preferred.
- Skills/Competencies:
- Strong clinical judgment and decision-making skills.
- Technical and analytical skills for analysis and interpretation of test procedures and results in order to determine appropriate actions and follow-up
- Interpersonal skills with the ability to work with and collaborate across the organization and with patients and their families as needed
- Ability to effectively lead, mentor and/or develop others
- Ability to maintain composure during challenging interpersonal interactions
- Proficiency in EMR systems. Epic preferred.
- Ability to manage a full patient panel independently.
